To ensure that your virtuous decision of taking a Birla Sun Life Insurance Policy serves your ultimate purpose please make a note of the following:
Ensure your beneficiary designations are updated and on file with us. This would ensure prompt claim settlement in the unfortunate event of death of the Life Assured.
Pay premiums regularly. This would ensure that Policy is in force. A lapsed policy would mean no death benefits payable to the nominee.
Keep a record of all your insurance policies and make a file of it. Maintain record of the Advisor / Branch with their address and contact particulars. Ideally share this information with your loved one.
Educate the loved one on the process to lodge a claim. It is the responsibility of the beneficiary to inform us about the claim.

Death Claim |

In case of unfortunate event of death of the Life Insured the following standard requirements need to be submitted: |

(In case the death has taken place outside India, the Claimant is also required to submit 'Death Abroad Questionnaire')

Legal evidence of title where there is no nomination, assignment or the policy has not been issued under Married Women's Property Act.

II. Unnatural Death Claim |
|
If a death of the LA is for any reason other than natural or due to illness, it is termed as unnatural death claim. In such cases, the following additional documents need to be submitted.
1 * First Information Report
2 * Post - Mortem Report
3 * Police Inquest Report
4 * Panchnama Report
In case of death due to accident, the above documents would be a necessity. News paper cutting's (if any) may be submitted.
* Note: Attestation of all the above documents by Branch Head / Relationship Manager.

Accidental Dismemberment Rider Claim |
|
This Rider pays part/ full Face Amount to the life assured, if the Life Assured accidentally looses certain specified body part/s (for more information please refer the Policy document for Rider Provisions) provided that:
Dismemberment occurs while the Rider is in force, and
Due proof is submitted to BSLI that dismemberment was caused solely by external, violent and accidental means.

It is required to be completed by the Life Assured. The Life Assured has to state facts regarding the nature of dismemberment, date of event of dismemberment, the particulars of doctors consulted by the Life Assured.

Questionnaire to be completed by the doctor/Hospital which treated the life insured for his injury / dismemberment
It is to be completed by the doctor who attended the life assured during his treatment for dismemberment. The medical attendant has to give details about the nature of dismemberment, his diagnosis, nature of deformity.

CI / CI 17 Rider Claim |
|
The Critical Illness Rider pays the Coverage Face Amount if the Life Assured is diagnosed to be suffering from any one of the four / seventeen illnesses specified in the Policy conditions (for more information please refer the Policy document for Rider Provisions). The Claim for Critical Illness Rider benefit has to be lodged by the Life Insured only after 30 days from the date of diagnosis. |

Critical Illness Claimant form : It is required to be completed by the person entitled to policy moneys. The claimant has to state facts regarding the cause of event, date of event, the particulars of doctors consulted by the insured.

Family Physician's Statement : It is to be duly completed by the Life Assured's family physician.

Hospital Treatment Form : The form has to be obtained from the hospital where the life assured was hospitalized for treatment of his Critical Illness.

| 1. To whom will the claim monies be paid? |
The claim monies will be paid to the beneficiary who generally is the nominee / assignee / appointee (in case of a minor) as mentioned by the Life Assured in the Application Form for Insurance |

| 2. What happens if there is no nomination on death of the LA? |
In such an eventuality a “Succession Certificate" will have to be submitted by the Claimant. A Succession Certificate is issued on application by a competent court on the question of the right to the property of the deceased. The Succession Certificate should specifically provide for disbursement of policy monies. If, however, the deceased has left a will, a probate of the will is required along with the copy of the will. |

| 3. What if there are two or more nominees, how will the Policy Monies be paid? |
If there are two or more nominees, then a joint discharge will have to be given. The claim proceeds would be paid jointly to all the nominees. Alternatively all the nominees can give a joint discharge for payment of claim benefits in favour of one nominee, in which case the claim proceeds would be made in the name of the designated nominee. |

| 4. If the nominee dies during the tenure of the Policy what is the next step the Life Assured should do? |
The Life Assured should nominate some other person in Place of the deceased Nominee under section 39 of the Insurance Act. |

Except in some type of Claims (eg Critical Illness Rider) where a waiting period is involved, all Claims including death claims should be intimated as soon as possible. |

The information regarding death of the Life Assured has to be informed by the claimant or near and dear ones of the Life Assured to the BSLI Branch Office or can intimated directly to the Claims Section at the address mentioned above. |
